INDIAN STUDENT CERTIFICATION FORMS 506
Parents of Native American students are required to fill out the Indian Student Certification Form 506 in accordance with the Indian Education Act. Please download the form and return to the Native American Resource Program, 97 W. Delavan Ave., Room 215. For more information, contact our office at (716) 816-3183.
