| Teachers with NYS Initial teaching certificates must complete a mentored experience in their first year in the teaching profession. A framework of these mentoring experiences is provided in section 100.2 (dd) (2) (iv) of Commissioner’s Regulations (district professional development plans).

| Click Here to download the form to be used by candidates who are submitting an application for a PROFESSIONAL teaching certificate. It is to be completed by the superintendent of schools or, if the mentored experience was copublic school, the principal or person in equivalent position with the school.
