Harassment or Bullying Accountability

DASA, Civil Rights Data Collection School Form, and SSEC Data Collection

This form is to be completed by the DASA Coordinator for each student involved in any reported incident of harassment or bullying within 48 hours of the complaint. 

Additionally,

  • If 911 is contacted, please make sure an incident form is completed and submitted. 

  • Please make sure you reach out to your ASL and call ED Wagstaff (716-818-5995).
