Required Immunizations for the 2026-2027 School Year

Avoid the back-to-school rush at your students’ doctor’s office. Schedule a visit now for their annual physical examination and to ensure they are up-to-date with their NYS mandated immunization requirements. Updated medical records must be received by 9/16/26 or else students without proper immunizations will be excluded from school starting September 17, 2026 in accordance to NYS Public Health Law 2164. Updated records can be sent to medicalrecords@buffaloschools.org

Exemptions from Immunization Requirements Effective June 13, 2019, PHL § 2164 only allows for medical exemptions to school immunization requirements. New York law does not recognize non-medical exemptions. Medical exemptions are defined in PHL § 2164 as: Medical Exemption. If a physician licensed to practice medicine in New York State certifies that an immunization is detrimental to a child's health, the requirement for that immunization is waived until such immunization is no longer detrimental to the child's health. The NYS licensed physician must complete the form link below. This form is to be submitted to your student's school nurse. This form must be completed annually and will be reviewed and approved by the BPS Medical Team.

Updated Immunization Requirements:
Students with an Individualized Education Plan (IEP) who are enrolled in the Extended School Year (ESY) summer school program must be up to date with all required immunizations for the grade they will enter in September.  Your child will be excluded from ESY if they're not up to date with their immunizations. Please make an appointment today.
